The problem is intermittent, and seems to be triggered by fast traveling, interacting with a door, being killed, or loading a saved game after the game has already been loaded. Sometimes I will get lucky, and if I do not get killed in-game, and if I avoid using doors, and I avoid using fast travel I will be able to play the game for 10 minutes without this problem showing up.
